Job Description

 Essential Job Functions:   - Computer entry; Enters patient name; Pt. ID number; Room number; Diagnosis; Physician; Allergies; Height/Weight; Age; Medical record number; Enters correct charge description. - Enters mg/kg/dose or mg/kg/day for all pediatric and neonatal orders. - Performs computer backup. - Enters correct medication:  Medication; Strength; Route; Correct expiration date; Posts debit/credits accurately; Start/stop date; Enter metric dosage. - Maintains orders entry error rate < 5%. - Serious order entry rate < 1%. - Capable of entering a minimum of 75 orders per eight hour shift. - Averages at least 50 orders entered per 8 hours. - Generates required reports: MAR, Pick I.V, production reports. Computer System: - Able to look up a patient's location. - Able to look up a patient's demographic information such as height, weight, allergy. - Able to run patient census and location lists. Pyxis Narcotic System: - Able to refill, load and unload medications. - Able to run reports. IV Preparation: - Performs calculations accurately. - Accurately completes IV rounds. - Maintains aseptic technique. - Checks for particulate matter in final product. - Only one product is prepared at a time. - Recycles product as appropriate. - Changes prep and expiration date on recycled product. - Dates, times and initials all vials for reuse. - Adds additives in the proper sequence. - Syringe and Vial count reflects what has been made. - Labels containers accurately including auxiliary labels. - Completes packaging including seals, protective wraps, etc. - Capable of preparing 250 non-pre-mixes per eight hour shift. - Prioritizes work (i.e. stats made first). - Delivers IVs to proper locations. - Restock shelves and rotates stock. - Properly disposes of non-recyclable solutions. - Maintains IV refrigerator temperature log. TPN Preparation: - Enters correct patient data. - Prepares TPN as needed. - Has prepares at least 30 units accurately, including additive, selection, measurement, labeling. - Logs correct solutions. Chemotherapy Preparation: - Prepares chemotherapy as necessary and documents. - Takes required protective measures. - Has prepared at least 30 units accurately, including product selection, measurement and labeling. Inpatient Dosage Preparation: - Fills orders with correct medication. - Prepares unit of use dosage when possible. - Keeps pharmacist informed of workload status. - Prioritizes work to prepare STAT, now and waiting in order of importance. - Follows through on fill on arrival orders. - Returns medications to stock and rotates stock. - Fills requisitions from nursing floors in a timely fashion. - Prepares outpatients prescriptions: - Processes prescriptions within seven minutes of start. - Completes Rx log. - Completes billing log. - Fills out pharmacy profile for employees and IMS patients. - Records Rx on patient profiles. - Prescription label and blank contains all legal requirements. Cassette Exchange: - Fills medication cassettes and delivers them on time. - Checks for computer entry errors and notifies the pharmacist. - Adjusts charges. - Less than 3 filling errors per 75 cassettes filled. - Transfers bulk and prn meds appropriately. Quality Improvement: - Continuously assesses and improves the performance of care and services provided. - Demonstrates knowledge of CQI through application of the principles in the department. - Uses systematic methods to improve services, plan changes and evaluate outcomes. - Demonstrates effective problem solving techniques and promotes same in others. - Actively supports the mission, goals and objective of the hospital and department through CQI activities. - Completes special projects on time.
Qualifications
- Minimum Education:  High school graduate or equivalent preferred.  Knowledge necessary to obtain Pharmacy Technician licensure in the state of California and current CA Pharmacy Technician Licensure. - Minimum Experience:  Experience in I.V. admixtures, unit does drug distribution and computer entry preferred. - Preferred Skills and Abilities:  Typing skills of 30 WPM; good communication skills, both written and verbal.
