Clinical Pharmacist
Job Description:
Under general direction, a Clinical Pharmacist performs professional clinical pharmaceutical duties to assure quality patient care, appropriate drug therapy and compliance with all applicable regulations. Essential functions include: monitoring drug therapy for appropriateness, contraindications, interactions, adverse effects and therapeutic/toxic dosing; effectively communicating information on drugs/medications to other health care personnel; reviewing, processing and dispensing physician medication orders, assuring drug therapy and compliance with all applicable regulations; performing quality-assurance activities, Pharmacy and Therapeutic Committee formulary drug reviews and patient specific pharmacokinetic monitoring; responding to inquiries about case management and referrals from physicians, nurses, pharmacists, other health care professionals and the public; conducting departmental in-services and other teaching/training activities; supervising support personnel; and actively participating in continual development and implementation of departmental policies, procedures and programs. The Clinical Pharmacist also performs other related duties as assigned/required.
Minimum Qualifications:
1. Possession of a valid Registered Pharmacist license issued by the California State Board of Pharmacy; AND ONE OF THE FOLLOWING:
2a. Completion of a one (1) year pharmacy residency program; OR
2b. Completion of a one (1) year post-graduate pharmacy fellowship program; OR
2c. One (1) year of professional level experience as a Clinical Pharmacist (equivalent to class 2454 Clinical Pharmacist); OR
2d. Possession of a Board Certified Clinical Pharmacist, Certified Geriatric Pharmacist or Board Certified Psychiatric Pharmacist certification.
