Job Description

Banner Baywood Medical Center pharmacy department in Mesa, AZ is seeking a clinical pharmacist to join our dynamic outpatient team in the Medication Management Clinic. This position offers a variety of ambulatory care opportunities with a focus on direct patient care in a collaborative practice environment. The clinical services provided within the clinic include a well established Anticoagulation Clinic, Blood Management Service, Heart Failure Clinic, and Ambulatory Treatment Unit. We have a progressive clinical practice with great physician and administrative support. Our campus is also home to Banner Heart Hospital and we have PGY-1 (general practice) and PGY-2 (cardiology) residencies. We are affiliated with the University of Arizona College of Pharmacy and Midwestern University College of Pharmacy-Glendale which offer precepting opportunities to our pharmacists. The clinic is continually looking for expansion opportunities to meet the diverse needs of our community.

 

This is a full time position working day shift Monday-Friday, hours will vary.

 
Required Education

Current pharmacy or eligibility for licensure in the State of Arizona, a degree in Pharmacy (Doctor of Pharmacy degree preferred) from an accredited College of Pharmacy, and completion of an ASHP accredited residency program are required. Previous clinical pharmacy experience is desirable. Anticoagulation Management Experience preferred. Must possess good organizational, problem solving, and computer skills. Qualified applicants should demonstrate excellent written and verbal communication skills to interface with all levels of Banner Health staff, physicians, and other contacts.
