St. Clare's Hospital of Weston, Wisconin - Position for 7 on and 7 off
The Staff Pharmacist performs a variety of professional pharmacist, clinical, administrative and distributive duties.
* Medication dispensing andistributive functions
*Medication checking: verification of medications prepared for delivery by technician staff. All products released are correctly labeled packaged and filled.
-Sterile and non-sterile packaging: verification of medication preparation by tech, including IV preparations, compounded products, batch products and oral syringes.
*Ensuring that STAT medications are delivers to the nursing unit in a timely fashion.
*Computerized Physician Medication Order Entry: Interprets and reviews medication orders for accuracy
*Clinical review of patient medication profiles: review of the patient medications profiles will occur during review of new orders and as other opportunities arise. Clarifies orders with the physician as needed and documents any interventions on inappropriate orders.
*Drug Dosing: Determining if dosage adjustments are warranted based on the patient's age, renal and liver function, or other pertinent clinical factors Perform those functions that adhere to, and support, the clinical initiatives of the hospital/pharmacy to include, but are not limited to: therapeutic formulary interchange, formulary driven cost containment initiatives, formulary management.
Qualifications: Pharm D or BS from an accreditied college or university of the U. S.
*Licensed Pharmacist (Registered Pharmacist) in the State of Wisconsin
*Preferrably preformed in a inpatient care setting
*Knowledge of the principles of growth and development and the ability to assess data and identify patients' age-specific requirements and
