The clinical pharmacist at Group Health works closely with the Primary Care and/or Specialty Teams on a daily basis. They focus on medication quality, safety, and utilization; patient education/service and Medication Use Management (MUM) initiative issues. They assist in managing select patient populations or disease states with "complex" medication related issues. They also partner and collaborate with pharmacy manager(s) and clinical pharmacist within their regional team in identifying, sharing and spreading best practices. Meet regularly as a member of the area clinical pharmacist team and attend clinical pharmacist meetings, pharmacy meetings and clinic team meetings where appropriate.
JOB DUTIES AND RESPONSIBILITIES
1. Provide timely and appropriate pharmaceutical care in collaboration with the patient, health care providers and pharmacy staff, assuming responsibility and accountability for designing, implementing and evaluating drug therapy.
2. Partner with appropriate primary care and/or specialty groups at local levels, including attending to team meetings where appropriate
3. Assist primary care and/or specialty teams in management of defined patient populations or disease states with complex medication-related issues.
4. Support primary care and/or specialty teams in meeting or exceeding Medication Use Management and Group Health endorsed clinical quality goals.
5. Participate in teaching classes, educational sessions, or planned group visits where appropriate.
6. Manage drug shortages or clinical safety issues in collaboration with Pharmacy Manager.
7. Enhance pharmaceutical care through mentoring and educating health care professionals.
8. Adhere to customer service standards that ensure all Group Health customers will be treated in a friendly, caring, courteous, and professional manner.
9. Communicate efficiently and effectively with target audience, including, but not limited to patients, physicians, nurses, and pharmacy staff.
10. Float to other areas of the pharmacy needing assistance when workload permits. Work on line as a distribution pharmacist as needed or as determined by pharmacy manager.
11. Respond in a timely manner to providers, patients, and nursing staff telephone inquiries.
12. Perform other duties as assigned or required. |
