Qualifications
Requires Registered Pharmacist licensed to practice in California. Pharm. D. and a Pharmacy Practice Residency Certificate (1 year experience), or 2-3 years equivalent hospital experience preferred.
Prefer completion of an accredited pharmacy residency program or one year of hospital based clinical pharmacist experience in the critical care setting.
Duties
Serves as clinical consultant, providing information to nurses and physicians regarding their patients. Common tasks include providing dosage recommendations for patients with renal impairment, toxicology information, antibiotic recommendations, special administration instructions, therapeutic interchange information, and advice regarding medication restrictions (e.g. formulary).\
Profiles medication orders for patients being admitted and awaiting an inpatient bed. Coordinates with the pharmacist responsible for the inpatient unit any special medication-related needs and /or outstanding follow-up on issues identified during the ED stay.
Reviews medication orders for appropriateness. When necessary, intervenes with the prescriber.
Documents clinical interventions on an ongoing basis. Measures the clinical and economic effectiveness of the service provided on an ongoing basis with formal reporting of the data at defined intervals set by management.
