The primary responsibility of the clinical pharmacist is to provide pharmaceutical care to patients receiving services at CCH. Computer order entry to medications, screening for appropriate dosing, providing drug information, and supervising preparation of medications by pharmacy technicians is required. Current NC pharmacy license is required. Collaborates with other healthcare providers for patient's continuum of care.
As such, the individual's responsibilities will include, but not be limited to the following activities: Prepares or supervises preparation of LVP,IVPB and hyperalimentation using aseptic technique. Prepares chemotoxic agents with proper technique. Dispenses pharmaceuticals accurately and in a timely manner. Checks IVs, Pyxis, surgery boxes, unit dose prepackaging, and other medications and assures their timely delivery. Responds immediately to initial consultations, drug level interpretation, and follows up with monitoring of drug therapy. Assures physician and nursing communications, both verbal and written, are accurate and complete. Supervises the work of pharmacy technicians and takes responsibility for medication preparation and delivery. Processes daily order to prime vendor wholesaler. Arranges for emergency shipments of medications from prime vendor wholesaler. Returns and credits IV solutions; restocks IV solutions as appropriate. Prepares daily IV order for materials management. Batches manual charges for data processing; corrects and rebatches charges rejected for faulty patient number or charge number. - Maintains controlled substance inventory by state and federal regulations. Enters and removes medications from the vault by department policies. Resolves all discrepancies during shift. Refills code carts, seals and documents on log; returns cart to nursing station. Completes expiration date check for assigned area and documents before the end of each month. Delivers patient profiles and MARs to nursing station. Demonstrates knowledge of special handling for chemo- toxic agents. Maintains Pyxis database, including resolving discrepancies, entering new formulary items, and modifying station inventory. Adds new users into Pyxis console and files signature form correctly. Uses professional judgment prudently with patient safety always given the highest priority. Demonstrates knowledge of, and consistently follows, department, hospital and corporate policies and procedures; Communicates appropriately and effectively with customers.
Qualifications BS from Pharmacy School, required. PharmD, preferred. Must have previous hospital experience. Current RPh license in NC. Must be able to work accurately and quickly. Supervisory skills. Verbal and written communication skills in English are essential to provide patient care. Basic computer skills (preferred). Understanding of pharmacokinetic dosing. |
