GENERAL DUTIES:
Perform clinical, distributive, supervisional, and educational functions necessary to support the departmental functions. Access to all Patient Health Information related to job function. Uses critical thinking skills and exercises independent decision-making as needed for the provision of safe and rational medication therapy.
DEPARTMENT SPECIFIC DUTIES:
1. Practices pharmacy in accordance with all applicable regulations and upholds pharmacy policy/procedures as appropriate
2. Accurately and efficiently assesses drug therapy in relation to patient specific conditions.
3. Establishes and maintains professional relationships with colleagues, physicians and other care providers.
4. Coordinates department operations and assures responsibility for professional development.
5. Establishes standards/norms for clinical practice/patient care and evaluates opportunities that improve patient outcomes.
6. Demonstrates independent leadership and professionalism. Qualifications Required:
Posses a valid CA pharmacist's license. Able to work all shifts and weekends. Excellent written & verbal communication skills. At least 6 months experience in a hospital pharmacy. BCLS
Preferred:
Pharm.D. completion of an ASHP residency program. Masters or other post-graduate degree, ACLS |
