Required Qualifications:

Licensed CA Pharmacist or committed to the CPGA exam

PGY1 or PGY2 Residency or 3+ years inpatient experience

Pharmaceutical care will include but is not limited to pharmacokinetic evaluation and monitoring, chart review, antibiotic monitoring, drug information, automatic renal dose adjustments, IV-PO switches, Coumadin consults, therapeutic interchange, and educational services, as well as reaching annual goals adopted by the Department of Pharmacy.
