Job Description

Job Duties Include: The Clinical pharmacist II is a licensed professional who is responsible for providing safe and appropriate pharmacy services to Walgreens-Option Care patients in accordance with the policies and procedures of the Walgreens-Option Care, pharmacy practice professional standards, and applicable regulatory entities. The Clinical staff pharmacist II coordinates patient services with the Nursing Department, monitors patient progress toward goals and educates patients, caregivers, staff and other professionals regarding appropriate medication use. Manage time effectively to fulfill practice responsibilities. Maximize work efficiency through the use of computers and other technologies. Speak knowledgeably of the scope of services that are available through Walgreens-Option Care. Determine the suitability of individual patients for home care. Design, recommend, monitor and evaluate patient-specific pharmacy care plans for the pharmacotherapy of home care patients. When appropriate, makes suitable recommendations for modifications in the prescribed medication therapy or monitoring parameters for a home care patient. Modify the pharmacy care plans for home care patients based on evaluation of monitoring data and other pertinent patient-specific information. Provide medication-use education to Walgreens-Option Care staff, other professionals, home care patients and/or their caregivers. Ensure continuity of pharmaceutical care to and from the home and other patient-care settings. Prepare and dispense medications using appropriate techniques and following Walgreens-Option Care's policies and procedures. Manage the use, maintenance, and troubleshooting of medication administration equipment and medication-related equipment used in the management of home care patients. Participate in the process for assessing, managing, and reporting medication-related incidents and equipment error and/or failures. Provide concise, applicable, and timely response to requests for drug information from health care providers and home care patients. Provide inservice education to physicians, nurses, pharmacy technicians, and other practitioners on home care pharmacy-related issues. Manage the use of investigational drug products according to established regulations and Walgreens-Option Care policies and procedures. Utilize pharmacy support personnel effectively. Participate in on-call responsibilities of the pharmacy department. In all activities comply with accreditation, legal, regulatory, and safety requirements.

Minimum Requirements:

  • Graduate of an accredited school of pharmacy.
  • Licensed and in good standing with the Board of Pharmacy.
  • Experience with and knowledge of sterile IV admixture procedures.
  • Board certified in a clinical specialty and/or has completed an accredited residency program in clinical pharmacy practice.
  • Excellent communication skills, flexibility and ability to work as part of a multi-disciplinary team.
  • Self-directed with excellent time management skills and able to work independently.
  • Current valid driver's license and proof of current automobile insurance.
  • Preferred Qualifications:

    Prior experience managing complex patients in the alternate site. Previous experience as a supervisor. Active member of a state or national professional organization.
