| Position Summary: This position is responsible for providing consultant services to skilled nursing (long-term care) facilities and assisted living facilities. Will manage appropriate drug therapy and review of patient medical charts in accordance with federal and state regulations and SeniorMed policy.
Evaluates physician medication orders for appropriateness of drug, dosage, potential interactions, route of administration and stability. Makes recommendations for therapy changes to the physician and facility staff as appropriate. Analyze patient medication regimens for appropriate medication usage.
Reviews the medication regimen and clinical indicators of each resident as assigned at least monthly for skilled nursing facility or quarterly for assisted living facilities and reports any irregularities to the Director of Nursing, or Nurse Coordinator of the facility, Administrator and the attending physician(s) with a written report of findings and recommendations.
Documents all communication with physicians, nurses, other members of the healthcare team and patients/residents and their family members
Acquires at least 50% of continuing education hours required for re-licensure in the area of geriatrics and/or state and federal regulatory updates as pertains to long term care facilities and/or assisted living/residential care facilities.
Communicates and manages relationships effectively with staff, patients, family members of residents and other members of the healthcare team.
Completes designated reports on time for distribution to the pharmacy.
Ensure that the pharmacy is in compliance with all local, state, federal and accrediting rules and regulations re: the practice of pharmacy, as well as Seniormed LLC's ethics, policies and procedures.
May assist Director of Consulting Services in staff education on utilization management concepts and activities, pharmacy staff training evaluation and development.
Provides articles/information for customer newsletters upon request.
Travels as necessary.
Provides excellent customer service to all external and internal customers.
