Provides consultative services including: anticoagulation, drug information, formulary reviews in services on pharmacology/disease state pharmacotherapy, pharmacokinetics to medical, nursing, and allied health professionals. Provides prescribed medication
Qualifications
Graduate of School of Pharmacy; BS in Pharmacy; PharmD preferred.-Must be a licensed pharmacist. Previous hospital pharmacy experience preferred. |
