Job Summary:
Serves patients by preparing compounds and dispensing prescribed medications, drugs, and other pharmaceuticals for patient care. Also provides pharmacological information to multi-disciplinary health care team and monitors patient drug therapy. Functions independently.
Essential Functions:
Oversees all phases of the activities relative to the provision of prescription products to patients served. (100%)
Interprets physician orders for correctness, reviewing for possible drug interactions/incompatibilities, transcribes new orders into information system, documenting clarification of orders with physicians and potential adverse drug events. Documents clinical interventions. (100%)
Dispenses medications by compounding, packaging and labeling pharmaceuticals. (50%)
Directs pharmacy technicians engaged in mixing, packaging and labeling pharmaceuticals. (50%)
Provides pharmacological information by answering questions and providing information to pharmacy customers and other members of the health care team on drug interactions, side effects, dosage and storage of pharmaceuticals. (20%)
Oversees or directs laboratory monitoring required for nutrition support patients. Evaluates all orders for nutrition support products. Consideration must be given to the appropriateness of the orders and the schedule for laboratory monitoring. (40%)
Assesses medications to determine identity, purity and strength. (100%)
Complies with state and federal drug laws as regulated by the State Board of Pharmacy, the Drug Enforcement Administration and the Food and Drug Administration by maintaining records for controlled substances; removing outdated and damaged drugs from the pharmacy inventory; maintaining current registration; studying existing and new legislation; anticipating legislation; advising management on needed actions. (100%)
Performs chart reviews as required for assessment of therapies, interactions and patient response to medications and for regulatory required monitoring of therapy parameters. (25%)
Knowledge / Skills / Abilities:
· Practice of institutional pharmacy

Minimum Position Qualifications:
· Education: Completion of 5-year course at an accredited School of Pharmacy leading to a degree of Bachelor of Science in Pharmacy
· Experience: Hospital/Health care experience; entry level individuals will be considered
· Training: Rotations required by Bachelor of Science degree plan; Appropriate competency assessments completed
· License / Certification: Current licensure to practice pharmacy in the State of Texas or be eligible for reciprocity into Texas from another state; CPR certification; driver's license and proof of current auto liability insurance.
Preferred Position Qualifications:
IV preparation course work in degree plan or IV preparation acquired certification
