STAFF PHARMACIST - HOSPITAL

JOB DESCRIPTION:

Provide drug therapy as well as patient and staff education for the purpose of achieving definite outcomes that improve a patient's quality of life.  Complete physician medication orders, monitor patient medication therapy through the patient profile, and provide medication information.  Cooperate with professional staff in designing, implementing and monitoring a therapy plan to produce specific therapeutic outcomes for the patient.  Responsibility for the assessment, treatment and/or care of patients (all age ranges) including neonatal, pediatric, adolescent, adult and geriatric.  Supervise supportive personnel and possible assumption of  managerial responsibilities in the Director’s absence of the director.

REQUIRED:

Current licensure as a registered pharmacist in the state of Texas.  License must be in good standing with the Texas State Board of Pharmacy.

Graduate of an ACPE accredited school of pharmacy

BS Pharmacy degree or Doctor of Pharmacy degree.  Hospital experience strongly preferred.
