In Patient Pharmacist Needed (full time, rotating night shift, hours will be determined)
**ONLY INPATIENT PHARMACIST WILL BE CONSIDERED FOR EMPLOYMENT**
Qualifications Requirements:             
·        Pharmacists shall be qualified to perform work in accordance with the standards of the Joint Commission and the Standards of Practice, American System Pharmacist. Pharmacist shall meet the following minimum qualifications
·        Be a graduate of an accredited college or university school of Pharmacy
·        Be licensed as a pharmacist with a current, valid, full and unrestricted license in one of the 50 states and/or the District of Columbia, the Commonwealth of Puerto Rico, or territories of the United States.
·        Have a minimum of 1 year of licensure with the last 3 years
·        Be in possession of: certified true copy of diploma issued to the individual for an accredited College of Pharmacy; certified true copy of the college transcript for an accredited College of Pharmacy; certified true copy of current license renewal certificate; position history covering the last recent 5 years; letter of reference from most recent employer
·        No records or convictions involving any form of chemical dependency, previous convictions of felonious nature, or any adverse actions pending by any Board of Pharmacy
·        Certification in Basic Life Support (BLS)
·        Current on all immunizations
 
Hours of Performance:  40 hours per week
Specific Tasks: Pharmacists shall perform essentially the same duties as those required of any military or government civil service pharmacist. Duties shall consist of but are not limited to:
·        Receive and review prescriptions for the full range of pharmaceuticals to include controlled drugs on the formulary. Determine type of substance, strength, dosage, and quantity to be dispensed in accordance with accepted practice and pharmacy standing operating procedures (SOP’s). Enter prescriptions into outpatient computer system, reviewing profile for allergies, drug/drug or drug/food interactions, and duplicate orders. Coordinate with prescriber to clarify issues or problems with dosing, interactions, or instructions. Obtain the pharmaceutical, prepares package labels, visually inspects the medication in the bottle to be dispensed, and verifies appropriate medication, checks the printed label affixed to the container against the original prescription for correctness. Provide instruction to patients on proper use of medications.
·        Prepare the full range of sterile products and performs pharmaceutical calculations for a variety of preparation. Responsible for the safe handling, preparation, storage, and disposal of cytoxic and hazardous drugs. Ensures strict adherence to sterile procedures and correct operation and maintenance of equipment. Receive and reviews, prepares, and fills physician orders for intravenous (I.V.) solutions, other parenterals, inpatient unit dose orders, prescriptions, and bulk drug orders. Perform final pharmacy check on sterile products, bulk drug orders, replenishment of emergency medication carts and inpatient unit dose medication carts in accordance with local policy. Perform advanced pharmaceutical calculations in the manufacture of intravenous admixtures and complex parental nutrition solutions dealing with body surface area and creatinine clearance rate formulas. Enter medication orders into inpatient computer system, reviews orders for allergies, drug/drug or drug/food interactions and duplicate orders
·        Receive and process orders for controlled substances from prescriptions, wards, clinics, and pharmacy subsections. Screen orders and prescriptions for accuracy, appropriateness and conformity to laws and regulation. Perform final pharmacy check and issues controlled substances to authorized areas, completes issue and control documents ensuring that chain of custody is maintained. Safe guard controlled substances according to regulations and pharmacy policies, reports all problems to the Division’s Senior Pharmacy Technician and /or Chief, Pharmacy Division.
·        Provide technical guidance to Pharmacy Technicians. Routinely refer to standard reference texts and journals to keep abreast of current trends, new drugs, and therapeutic indications
·        Performs other duties as assigned
