PHARMACIST - INPATIENT PHARMACY
GENERAL SUMMARY:
This position screens physician orders to ensure the correct medications are given to individual patients that range in age from birth to 100+ years assuring that the most correct drug and dose is prescribed, dispensed, administered and properly monitored. Screens orders for proper drug/ dosage, drug interaction, adverse reactions, allergies, and contraindications. Researches and answers drug information questions and provides various clinical pharmacy services.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
1. Receives and reviews all prescriptions for correctness and appropriateness and dispenses the most appropriate medications for individual patients.
2. Doses patients on any medications provided by the kinetics service and follows kinetic policies and procedures. Manages dosing of renally eliminated drugs and TPN.
3. Responsible for the pharmacy workflow and leadership on their shift and communication between shifts. Follows up on problems during the course of the daily shift.
4. Supervises the preparation of intravenous admixtures and parenteral products.
5. Provides and documents drug information to healthcare professionals and patients.
6. Assists in the maintenance of the hospital formulary by intervening on non-formulary orders and helps with the maintenance of the pharmacy inventory. Recommends antibiotics, when to switch patients to oral therapy and advises on duplicate therapies.
7. Involved in teaching of patients, students, or other healthcare professionals, i.e., discharge counseling, inservices, and hands on teaching of students. Also involved in the training of new pharmacists and pharmacy interns and residents.
8. Assists in formulating new/revised pharmacy policies and procedures through actual revision or participation in performance improvement teams.
9. Participates in patient care rounds with nursing, physicians, dietitians, social workers and other health professionals to provide multidisciplinary approach to the care of each patient.
MINIMUM QUALIFICATIONS:
1. Bachelor’s Degree in Pharmacy and current licensure in the State of Michigan required. Completion of a 6 year Pharm. D. degree preferred. Annual completion of 15 hours of continued education required.
2. One (1) to three (3) years experience working in a hospital or medical center pharmacy setting preferred.
3. Basic knowledge of personal computer needed to enter prescription information.
4. Organizational skills needed to prioritize multiple tasks.
5. Interpersonal skills needed to communicate successfully with individuals and groups and interact with people at all levels to communicate ideas and concepts in a clear and understanding manner.
6. Provides for the age specific needs of the population served according to department standards and policies/procedures as evidenced by observation, documentation, and peer feedback for the following: Neonate/Infant, Toddler, Child, Adolescent, Adult and Later Adult.
7. Mandatory tuberculosis (TB) testing must be done annually.
8. Mandatory Occupational Safety and Health Administration (OSHA) training must be completed annually according to the Exposure Control Plan.
PHYSICAL/MENTAL REQUIREMENTS AND TYPICAL WORKING CONDITIONS:
1. Must be able to lift 20 lbs.
2. Overall dexterity is required including handling, reaching, grasping, fingering, and feeling.
3. Ability to handle multiple tasks and stressful situations.
4. Overall vision is necessary, including reading and close up work.
5. Hearing and speech on the phone as well as in person are necessary.
6. Must be able to communicate verbally in person, on the phone and in writing.
7. Must be able to freely move around the department or hospital.
8. Frequently position requires regular public contact, decision-making and exposure to stress.
9. Frequently required to sit for long periods of time.
10. Some exposure to blood borne pathogens and other potentially infectious material.
11. Must be able to work in a latex environment.
REPORTING RELATIONSHIPS:
Reports to the Clinical Coordinator or Clinical Supervisor of Pharmacy Services.
