Staff Pharmacist Position With The University Of New Mexico!!!!!


Job Description
JOB SUMMARY
Provide specialized Pharmacotherapy and Clinical Pharmacology services to patients receiving care within his/her specialty area. Serve as a specialty resource to primary providers. Develop and execute pharmacologic research and participate in various educational activities. Ensure adherence to Hospitals and departmental policies and procedures. Patient care assignment may include neonate, pediatric, adolescent, adult and geriatric age groups.

ESSENTIAL FUNCTIONS
Essential and other responsibilities and duties may include but are not limited to the following:

 

PATIENT CARE - Thoroughly reviews patient's medical therapy through participation in patient care rounds, review of medical and medication administration records and discussion with other health care providers to assure optimal pharmaceutical care

PATIENT CARE - Provide pharmacologic and pharmacotherapy information to health care professionals, as requested or as necessary; this information may be formal or informal, written or unwritten

QUALITY IMPROVEMENT - Continually evaluate current activities and propose new areas of involvement or changes in current practices

PATIENT CARE - Document all interventions and activities in departmental records as requested

REPORTS - A monthly, semiannual and annual report summarizing cost containment and quality improvement activities will be submitted

PATIENT CARE - Document all medication errors and adverse drug reactions in accordance with Hospital policies and procedures

PATIENT CARE - Conceptualize, develop, solicit and gain all necessary approval for and execute at least one pharmacotherapy or pharmacoeconomic research project annually

GRANTS - When applicable, obtain grants from outside sources to cover the cost of the research

PATIENT CARE - Provide overall support to departmental and Hospital research activities through collaboration with other department members and provision of backup support and coverage

PATIENT CARE - Actively participate in the department's consultation services and provides pharmacology/pharmacokinetic consults, as necessary

PROJECTS - Complete two or more Medication Utilization Evaluation projects annually and provides expertise and consultation to the Pharmacy and Therapeutics Committee

RESEARCH - Publish research findings in a peer-reviewed professional journal

RESEARCH - Present research findings in a state or national professional meeting

TRAINING - Provide training and education to Pharmacy and Medical staff, Medical residents, Nursing staff, Nursing students, or other health care professionals concerning pharmacotherapy or related topics

EDUCATION - Participate in the education of Pharm. D. students at the UMN College of Pharmacy

EDUCATION - Retain the appointment of Clinical Adjunct Professor at the UNM College of Pharmacy

TRAINING - Precept students and provides lectures at the College

PROFESSIONAL DEVELOPMENT - Review the medical, pharmacology, and pharmacy literature consistently to assure knowledge of recent and significant developments

COORDINATION - Actively attends and participates in Hospital and Medical Staff committees, as assigned

OTHER DUTIES - Perform other duties as assigned or requested

SUPERVISION- Organize and guide the daily operational duties of the support personnel

PATIENT SAFETY 1 - Follow patient safety-related policies, procedures and protocols

PATIENT SAFETY 2 - Demonstrate proactive approach to patient safety by seeking opportunities to improve patient safety through questioning of current policies and processes

PATIENT SAFETY 3 - Identify and report/correct environmental conditions and/or situations that may put a patient at undue risk

PATIENT SAFETY 4 - Report potential or actual patient safety concerns, medical errors and/or near misses in a timely manner

PATIENT SAFETY 5 - Encourage patients to actively participate in their own care by asking questions and reporting treatment or situations that they don't understand or may "not seem right"

 MINIMUM QUALIFICATIONS

EDUCATION:
Registered Pharmacist with the State of New Mexico
LICENSES/CERTIFICATIONS:
Licensed Pharmacist in State of New Mexico
CPR Certification or must obtain within 30 calendar days of date of position

 PREFERRED QUALIFICATIONS

 PREFERRED EDUCATION:
Master's Degree of Science clinically oriented in Pharmacy, Doctor of Pharmacy Degree, or Pharmcotherapy Board Certification

PREFERRED EXPERIENCE:
1 year level II pharmacist experience, as determined by the review committee

 WORKING CONDITIONS

 ENVIRONMENTAL CONDITIONS:

Subject to UNM credentialing process; must maintain credentialed status
PHYSICAL CONDITIONS:
Medium Work: Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Light Work.
