Clinical Pharmacist (Columbia, SC)- 002922

Description

·         Ensures compliance with federal, state and local laws and regulations, Coram Policies and Procedures (P & P) and standards of accrediting body.
·         Obtains, interprets and evaluates all physician medication orders for therapeutic appropriateness, cost effectiveness, accuracy, safety and completeness.
·         Initiates, reviews and updates all required documentation per Coram P&P.
·         Accurately maintains all legally required records.
·         Communicates clinical and technical information and monitoring to other members of healthcare team and patient when appropriate.
·         Participates in patient evaluation for acceptance, product selection, device selection and supply management.
·         Performs accurate computer order-entry, labeling and dispensing in accordance with Coram P&P and state board of pharmacy regulations.
·         Accurately compounds and checks compounded sterile products according to Coram P&P.
·         Directly supervises pharmacy support personnel in the appropriate compounding, packaging and labeling of compounded sterile products.
·         Follows established procedures for drug and supply procurement.
·         Maintains appropriate inventory levels and ensures use of preferred products per Coram formulary.
·         Maintains procedures for appropriate security of all legend items, including controlled substances
·         Participates in on-call responsibilities.
·         Participates in Performance Improvement activities, educational programs and departmental/branch meetings.
·         Participates in the branch assessment process as requested by pharmacy manager or designee.
·         Communicates appropriately with all members of the healthcare team.
·         Assists in training and orientation of new employees as requested by pharmacy manager or designee.
·         Performs other related duties as directed by supervisor.
 
Qualifications
·         Bachelors or Doctor of Pharmacy Degree in Pharmacy from an accredited college or university
·         One year related experience in an IV alternate site environment or equivalent is preferred
·         Ability to read, analyze, interpret general business periodicals, professional journals, technical procedures, or government regulations.
·         Ability to write and present information to managers, patients, nurses and physicians.
·         Computer skills (word processing, database, spreadsheet programs)
·         Current license to practice pharmacy in state of employment. 
·         Current CPR certification if involved with direct patient contact. 
PHYSICAL DEMANDS
·         Regular talking and hearing.
·         Requires the regular use of hands and fingers to mix medications and operate equipment.
·         Regular standing, walking and sitting.
·         Vision requirements include close vision, color vision, night vision, peripheral vision, depth perception and the ability to adjust focus.
·         Must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. 
WORK ENVIRONMENT 
·         While performing duties of this job, the employee is frequently exposed to toxic or caustic chemicals.
·         The employee occasionally works near moving mechanical parts and is occasionally exposed to fumes or airborne particles.
