Job Description
We are currently seeking a well qualified clinical pharmacist to practice in the medical/cardiac intensive care unit (full time, 7a-3:30p, M-F, occasional weekends, and holiday rotation). The clinician in this role will be responsible for serving as a professional resource for adult critical care pharmacotherapy. Responsibilities will include, but are not limited to: providing direct patient care through multidisciplinary rounding, reconciling medications, prospectively optimizing pharmacotherapy and serving as a drug therapy consultant to medical and other intensive care staff. This candidate will also be responsible for medication use evaluations, representing the department on hospital wide and unit specific committees, serve as a resource for other pharmacists, support day to day pharmacy operations and participate in projects that improve quality of care and/or result in overall cost reduction. Requirements: NH Pharmacist license, Pharm.D. or B.S. Pharmacy (Pharm.D. with residency preferred), ICU training and/or experience required.
We are a 209 bed licensed facility. Our hospital is the largest critical care facility on the New Hampshire seacoast. Portsmouth is one of the most beautiful, cultured and diversified towns in New England. It is located directly on the ocean with easy access to Boston, Portland and the White Mountains.
Following physicians' orders, the Pharmacist compounds and dispenses medications and other pharmaceuticals. Position tasks and responsibilities include:
Maintains strong knowledge of hospital policies and government regulations and dispenses accordingly.
Consults with physicians and nursing staff regarding patient reactions, errors and complaints.
Determines and consults on possible drug interactions.
Assists in the supervision and training of pharmacy technician personnel.
Participates in Drug Utilization Review.
Assists with quality improvement initiatives, inventory and ordering of pharmaceuticals.
Monitors, records and communicates as appropriate utilizing computerized documentation systems and order entry.
Utilizes knowledge of age specific needs of patient in performance of duties and responsibilities.
Orients and mentors new staff members.
Follows Standard Precautions using personal protective equipment as required.
Required Job Qualifications include:
Graduate of a college of Pharmacy accredited by the American Council on Pharmacy Education.
BS or Pharm.D degree.
State licensure as a Pharmacist.
Customer service abilities including effective listening skills.
Critical thinking skills, decisive judgment and the ability to work with minimal supervision in a fast paced environment.
Ability to perform work that requires frequent standing, bending, reaching, squatting, kneeling, moving, lifting of patients and/or equipment up to 50 lbs.<br /><br />
